Healthy Texas Women Program Formulary File Layout

This layout document describes the fields in the HTW Program Formulary file. The file contains all drugs on the Healthy Texas Women Program and Healthy Texas Women Plus Program formularies. The file contains two tabs, one for each program.

Drug NDC Identifies the 11-digit National Drug Code (NDC) number 11    
Drug Name Identifies the name of the drug or product 35    
Drug Description Identifies the First Databank (FDB) label name of the drug or product 35    
Drug Package Size Identifies the package size of the drug 12    
Drug Unit Identifies the unit of measure of the drug 2  
  • EA = Each
  • GM = Gram
  • ML - Milliliter
Drug Legend Status Identifies whether the drug is legend or over-the-counter 10  
  • Non-Legend = Over the counter
  • Legend = Prescription required
Drug Effective Date Identifies the effective date of the drug on the HTW or HTW-Plus formulary 10 MM/DD/CCYY  
Drug End Date Identifies the termination date of the drug on the HTW or HTW-Plus formulary 10 MM/DD/CCYY  
Drug % Limit Refill Identifies the refill utilization percentage 3    
Market Basked Description Identifies the drug or product's market basket description 50
